• Dietary preference/allergies
• Medical allergies
• Emergency contact info
• Personal vehicle make/model/year/color and license plate – please put this info in the “comments or special needs” section (if you don’t have a personal vehicle, put “not applicable” in the comments or special needs section)
• The “travel” section is just for incoming core staff. Please select “no travel” if you are already here.

Please select the closest thing to an accurate arrival and projected departure date. If you’re not sure of your departure date, select one year from now as a default.
